Homebound Parent Information

Homebound services may be available If your child has a illness or a injury that prevents them from being able to attend regular classroom instruction for a minimum of 10 consecutive days and the child is under the care of a certified medical physician.

Frequently Asked Homebound Questions

 

 

Q.  What is Homebound?

A.  Special Program designed to serve students that are unable to attend school due to health reasons. A certified teacher will serve as a liaison between the student and the student’s classroom teachers.

 

  

Q.   Why would my child need to go on Homebound ?

 

A.       If your child could not attend school due to health impairments, such as a recent surgery, student is experiencing mental health/emotional issues AND under the care of a psychologist/psychiatrist, student is pregnant and experiencing complications. 

 

 

 

Q.     Who can qualify for Homebound?

A. Any student in the Milan Special School District

 

 

 

Q. Will my child be counted absent from school while on Homebound?

 

A. No

 

 

 

Q. How do I sign my child up for Homebound?

 

A. Student must be certified by a Medical Physician  (not a Nurse Practioner) as being health-impaired                                    

 

     and unable to attend regular instructional programs at school.

 

 

 

Q. How long does my child need to be absent from school before he/she is

 

    eligible for Homebound?

A. The student must be absent for a minimum of ten (10) consecutive days.

 

 

 

Q. How long can my child be on Homebound?

 

A. As long as the Medical Physician deems necessary.  If the student is going to be absent for more than twelve (12) consecutive weeks, the doctor must explain on the Homebound medical form. 

 

 

 

Q. Where do I get the paperwork for the doctor to fill out for me to start

 

     receiving Homebound services?

A. Paperwork for Homebound may be picked up at the Milan Special School

 

    District Central Office, located at, 1165 South Main Street,  Milan.

    Office hours are Monday-Friday 7:30 a.m. - 4:00 p.m.

 

 

 

Q. Who will come to our house and provide Homebound services?

 

A. A certified teacher will serve your child. The teacher may or may not be

 

     one of your child’s teachers.

 

 

 

Q.     How often will the teacher come to our house?

A. The teacher will provide your child with up to three (3) hours of service a week.

 

 

 

Q.     How will the Homebound teacher know what my child is missing at school?

A.   The Homebound teacher will work with the classroom teacher to get assignments and to return the work the student has completed.

 

 

 

Q. What if my child is too sick to have the Homebound teacher come visit

 

      that day?

 

A.     It is the responsibility of the parent/student to notify the Homebound

 

Teacher and inform him/her of the problem. Please notify the teacher in plenty of time.

 

 

 

Q.     Will I be graded on the work I do while on Homebound?

A.  Yes, it is the student’s responsibility to complete the work that the Homebound teacher brings to the student.

 

 

 

Q. What about my books, how can I get them from school?

 

A. If you know ahead of time that you are going to be out, please take

 

all of your books you are going to need. If you do not know ahead of time either your parents or the Homebound teacher can get your books for you.

 

 

 

Q. Does there need to be an adult at home when the Homebound teacher

 

     comes to the house?

A. Yes, a parent signature is required each time the Homebound teacher visits

 

 

 

Q.     Can a nurse practitioner sign my Homebound form?

 

A.     No, A medical physician must sign the form

 

 

 

Q. Can a mid-wife sign my Homebound form?

 

A. No, A medical physician must sign the form

 

 

 

Q. If my child gets pregnant while still in school, how will her services be

 

     different from regular Homebound services?

 

A.     There is a different form that pregnant Homebound student’s need to have

 

the doctor to fill out. Six weeks is allowed after the birth unless the physician certifies in writing that she is having health complications arising from the pregnancy that prevents her from returning to school after the baby is born.

 

 

 

Q. Can my child work while on Homebound?

 

A. NO

 

 

 

Q. What if we have a dog and the teacher is afraid of the dog.

 

A. Homebound teachers have the option of providing services to the student

 

     at the public library. This would be at the teacher’s discretion.

 

 

 

Q. What should I do if the Homebound form states that I should return to

 

      School on Tuesday, March 9 and I’m not able to go back until Friday,

 

      March 12 (just an example)?

 

A.     The student will be considered absent for March 9, 10 & 11. Unless

 

a doctors statement is brought to school to excuse those days. If a doctor’s note is not available the student will be considered unexcused.

 

 

 

Q. What if the doctor feels that it is necessary for me to be out longer than    

 

       he/she had originally planned?

 

A.     A new Homebound form will need to be picked up and filled out               

 

completely by the medical physician and then returned to the Homebound office.
